To complete your registration, you must agree to all of the following statements:

· I am supporting a young adult(s) age 12–25 who struggles with their mental health.
· I am not a young adult who is dealing with my own mental health difficulties.
· I understand that these are educational workshops and not therapy.
· Peer Facilitators may offer real life examples of how to use skills but cannot provide advice.
· Specific outcomes related to participating in these workshops cannot be guaranteed.
· Program materials are provided for personal use only and are not for redistribution.
· I agree to being identified only by my first name in the workshop.
· I will keep personal information that may be revealed during the workshop confidential.
· I agree that Lifeboat Family Skills, and any not for profit partners can use my contact information (Name and Email
Address) for the purpose of inviting me to join their mailing list so they can notify me about upcoming dates for programs and services they offer.
· I agree to hold Lifeboat Family Skills Limited and its employees, contractors, and partners harmless from all liability.
